Clinical AI Specialist

On-site in central Madrid · Full-time

This role doesn't exist. Yet. And still, it will be key in the coming years across every healthcare company.

Hermet was born with a clinical focus from day one. María, our AI for companionship and prevention dedicated to elderly people, needs clinical experts to keep learning and improving with every conversation.

You'll work alongside our CPO, Lorena (a nurse specialized in mental health), designing how María responds, accompanies, and detects.

You'll be the first person we hire for the clinical team, so your work will directly impact Hermet's evolution over the coming months and years.

What you do will become real conversations with elderly people within weeks. Every protocol you design, every exercise you create, every signal you define will shape María's behavior.

What you'll do

Design María's conversations
Define what she asks, in what order, how she adapts based on responses. Create conversational flows adapted to elderly interaction, including cognitive stimulation and routine tracking.
Evaluate the multi-agent system's clinical behavior
Listen to real conversations and identify clinically significant moments: when María needs to support through grief, sustain a difficult situation, or detect that something isn't right. Define how she should respond and translate it into concrete improvements.
Cognitive stimulation within a natural conversation
Design exercises based on reminiscence, orientation, verbal fluency, calculation, and memory techniques that integrate naturally into a conversation. Grade them by difficulty and adapt them to different profiles and stages of cognitive decline.
Define which signals matter
Establish which changes in language, emotional state, conversational behavior, or routines are clinically significant. Design the criteria for the alert system (cognitive, emotional, behavioral, and functional) and María's behavior when she detects something concerning.
Design the agent's safe boundaries
Define which situations fall outside María's scope and how she responds when she detects them: how she acts, how she safely steers the conversation, and what information reaches the care team.
Build the evidence
Help create clinical impact metrics. In the medium term, participate in publishing results, contributing to the evidence on the use of AI in elderly companionship.
Coordination with the technical team
Work in fast design-test-improve cycles with the technical team. Explain clinical judgment in a way that can be implemented.

What we're looking for

Essential:

  • At least 2 years of direct experience with elderly people and practical knowledge of cognitive stimulation and cognitive decline.
  • Ability to explain and structure clinical judgment so a technical team can implement it.
  • Ability to define what a good clinical response is and what isn't, in a way that can be measured and reproduced.

Highly valued:

  • Experience with group or structured cognitive stimulation programs.
  • Knowledge of cognitive assessment tools like the MoCA, MMSE, or verbal fluency tests, and ability to adapt their principles to conversational format.
  • Training or knowledge in clinical research methodology.
  • Having used generative AI tools in practice (ChatGPT, Claude, or others) and curiosity about how they work.
  • Experience or interest in health technology.
  • Professional English.

Personal profile:

  • You prefer building to following manuals.
  • You're comfortable with the uncertainty of an early-stage startup: not everything is defined, and that's part of the appeal.
  • You're genuinely motivated to improve the lives of elderly people.

What we're not looking for

  • A purely academic profile that needs highly structured environments.
  • Someone looking for a traditional clinical practice with their own patients.
  • A profile that isn't comfortable iterating quickly or working with technology.
